Our Residential Services for Adults

UCP of NEPA currently operates seven Community Living Arrangements where adults with developmental disabilities live in their home in their community. These residences provide 24-hour support while encouraging independence, personal choice, and active participation in community life.
Each person works closely with family members or advocates, their Supports Coordinator, and the UCP of NEPA team to develop an Individual Support Plan. This plan reflects their personal goals and outlines the supports needed to help them succeed, including medical, educational, vocational, and recreational services.
During the day, many individuals work, attend vocational programs, or take part in community-based activities. They are also supported in participating in household responsibilities and daily living tasks, while enjoying recreational opportunities that match their interests.
At UCP of NEPA, the goal of our Community Living Arrangements is to support each individual in building independence, strengthening connections within their community, and enjoying a fulfilling quality of life.

UCP of NEPA’s Supported Independent Living Program helps adults with disabilities live as independently as possible while remaining in the comfort of their own homes and as active members of their communities.
Participation in the program is based on each individual’s goals, preferences, and readiness for independent living, and is determined collaboratively with the individual, their family or advocates, Supports Coordinator, and the UCP of NEPA team.
Our staff are here to support each person every step of the way. This may include assistance with finding accessible housing, making necessary home modifications, and developing essential independent living skills. A Supported Living Coach provides individualized guidance in areas such as self-care, household tasks, budgeting and paying bills, and overall home management.
At UCP of NEPA, our goal is to empower each person to build confidence, strengthen independence, and live a full and meaningful life in the community they call home.
