Home Services for Children and Adults

Because many of the needs we support involve medical or health-related concerns, UCP of NEPA’s Home Services Program is staffed by experienced registered nurses/LPNs.
Our Home Services’ nurses bring both clinical expertise and a strong background in human services, allowing them to compassionately support individuals and families through a wide range of challenges.
For children, services may include helping families obtain appropriate evaluations, access needed equipment, and connect with supports that promote success at home, in school, and in the community.
For adults, support may include identifying and securing home modifications that promote independence, navigating available government programs and benefits, and connecting individuals with resources that enhance daily living and overall quality of life.
At UCP of NEPA, we are committed to meeting people where they are and helping them find the support they need to live healthier, more independent, and more fulfilling lives.

UCP of NEPA’s Home Services Program proudly serves individuals and families across seven counties in Northeastern Pennsylvania, including Lackawanna, Susquehanna, Wayne, Pike, Wyoming, Sullivan, and northern Monroe counties.
Home Services serves as a vital link to community resources, information, and support. We help individuals and families connect with services that promote independence, improve quality of life, and provide assistance for needs that might otherwise go unmet.
At UCP of NEPA, we are committed to ensuring that help is accessible, compassionate, and close to home for the communities we serve.

At UCP of NEPA, we are here to walk alongside individuals and families, offering support, guidance, and resources every step of the way.
During the initial home visit, a registered nurse completes a comprehensive assessment to better understand each individual’s and family’s needs, goals, and concerns.
Working in partnership with the individual and family, the nurse helps develop a personalized plan of support and connects them with community resources and other agencies as needed.
Follow-up home visits are provided based on each family’s needs, offering ongoing support and guidance. These visits help individuals and families navigate new challenges, access additional resources, and continue making progress toward their goals.
Through compassionate, person-centered care, we remain committed to empowering individuals and families to live with greater independence, dignity, and opportunity.
